Community Health Assessment

Community Health Assessments are conducted in partnership with Appalachian District Health Department and local Healthy Carolinians in Alleghany, Ashe, and Watauga Counties every four years. The Last Assessment was conducted in 2003 and the next assessment is scheduled to be completed by December, 2007.

The Community Health Assessment provides useful information from local community members about their health concerns, access to care, and individual health status.

The information collected is used to drive the work of local health departments and Healthy Carolinian partnerships and to address community concern and monitor our progress towards the 2010 Healthy Carolinian goals.
